APPLICATION FOR SHELTER RESERVATION

INSTRUCTIONS: Complete all information requested. Send the entire application directly to the park's address along with a check or money order for the Reservation Fee. Reservations are not final until they are confirmed by the park office and confirmation notice sent to the applicant. Reservation fees are $25 (except at Spitler Woods on Fridays & Saturdays after 4pm, the fee is $150).

PLEASE NOTE: If alcoholic beverages are sold, an additional permit and DRAM SHOP
insurance in the amount of $50,000 per person per occurrence is required. See Site Superintendent
for additional details and information.

Shelter reservations are subject to all statues and administrative rules governing the
park, which are posted at the site.
Shelter users are required to remove all belongings and place all trash in appropriate
receptacles.
Additional site-specific rules will be sent with your confirmation notice. Any violations
will result in the revocation of this permit, denial of future requests, and a $50 disposal fee.
Reservations are accepted by mail and in-person only. No reservations are accepted
over the phone.
Reservation Fee ($25.00 or $150 for Spitler Woods on Fridays & Saturdays after 4pm) for one day, must accompany application.
Reservation Fees are non-refundable unless area is closed by the Department of
Natural Resources.
